New Patient Form Packet includes the following items:
- Patient Intake Form
- Current Medical History
- Padnet Questionnaire
- Authorization from Patient or Legal Representative
Filling out the “New Patient Forms” before arriving to the office is a time savor for you!
Patient that have complete their paperwork should arrive 15 minutes before their appointment time. If you have not completed paperwork please arrive 30 minutes early.
